Use of the Contact Record Form
As a reminder, Investia’s Client Contact Record Form (F51-331A) (CCRF) is a form that must be used for note-taking purposes only and must always be uploaded to head office for imaging.
In the interest of efficiency, please be advised that effective Monday, April 28, 2025, all CCRFs submitted for processing will be automatically redirected to imaging without intervention from head office, whether or not they contain transaction instructions.
If transactions require head office action, please ensure that you use the appropriate Order Instruction Form and send it for processing. This will help eliminate transaction duplication errors and ensure efficient use of the Business Tracking Tool.
Changes to Order Instructions Forms
Following this change and in order to make it easier for you to use our forms, improvements have been made to our four Order Instruction Forms (OIFs): Purchase (F51-161A-5), Redemption (F51-161A-6), Switch (F51-161A-4) and Internal Transfer (F51-161A-7). Here are the details:
Addition of compliance questions
For those who found the questions on the CCRF useful, the mandatory questions regarding the type of transaction have been added to the OIFs:
Void cheque
The box for affixing a void cheque to add banking details to Univeris has been removed from the form (Purchase and Redemption OIFs only), as banking information must now be uploaded separately and exclusively via the Banking Information Management tool on the Advisor Centre. However, the “OIF – Purchase” still allows for the cheque associated with the specific purchase transaction to be added.
Addition or change of banking information
The banking information section has been updated on the form (Purchase and Redemption OIFs only) to specify that, where applicable, the details of your client’s bank details and signature must be included on the form.
Removal of the standard OIF
Following this update, please be advised that the standard Order Instruction Form (F51-161A) will be deleted and removed from Univeris effective Monday, April 28.
Availability of updated forms in Univeris
Amended forms will be available in Univeris on Monday, April 28.
